Reg .VSCT in Visual Studio

Am not sure if am repeating the question. I heared ab .VSCT for the first time and i googled about it with few MSDN links. Still am not getting clear picture of it Except (text file that describes the set of commands that a VSPackage contains and it is an XML File)But i had couple of questions like
  • where is the file located or we need to create it ?
  • where we can use any real time Examples ?
Hope i ll get some clear idea here :)thanks
