label and alignment problem

i got label,alignment problem as if any body modified the generated HTMl code from following link will be very grateful<br />

i want text field comes under Enter Text and drop down exactly under LABEL(HV) in same alignment and in UNTERVORGANG field one drop down comes ---please select--<br />

so when u select u get some fields i want label above each field <br />
<br />
if anybody help me out is appreciable<br />
