This is calculated as how oftenly does a keyword appeared in a site's title or description. You don't want to go overboard with frequency, however since on some engines if you repeat a word too many times, you'll be penalized for "spamming" or keyword stuffing.
In general though, repeat your keyword in the document as many times. Dealing with keywords are very easy as long as you connect it in quality content or sentences without doubting it was a spam. If you are using your keywords in title or title is made of keywords then it is not good and if you repeat those same keywords in meta description as well then it is surely keyword stuffing. For whole body do not make your keyword density more than 3% of your total words in an article or post. Keep the simple rule of KISS with you (keep it short and simple) when making title and description.
