How do i make a php send to mysql?

OK I need to do something verysimple. I need to make my php page send some information to a mysql database. I have searched else where but seemed to fined to much information to look through. Here is what i need it to do. I am need it to store ip addresses so that they can be recalled via $ip and they need to be stored into a database so they can be recalled at anytime and i want to use a form to do it with. I know this must seem very simple to you all but I am a newbie and I need some help.
