I think it could be because of a modification you have added. I think its the 'Thank you hack' Remove it and the error will remove

ya i know this is bcz of hack but i want thnak you hack work properly without this error how to do that?

It looks like the install failed because it didn't create the database table it need to track them. I think its not the thank you hack but one of the stat mods, looks like its cyb advanced stats or vS-Hide Hack Resurrection. I would reinstall it and overwrite what's there. Make sure you have the right script when you do the reinstall.
