Installation:
Upload the guitarhero.php, and the gh3widget.swf files to your forums root directory.
Then upload the product.

To add a link to the navbar:
Find:

Add Above:

Code: 
<td class="vbmenu_control"><a href="guitarhero.php$session[sessionurl_q]">Guitar Hero</a></td> 
Next, go into your AdminCP and under your vBulletin Options, Find the Guitar Hero settings and edit them to your liking.
 
Are you kidding? Works perfectly, so either you've installed it in the wrong location or you've just not installed it properly.
