form tag attribution

I want to add tag to an object, for example user1------ submituser2------ submituser3 ------ submit\[code\]user\[/code\] is just text which represents user list in my database, \[code\]-----\[/code\] represents "input area", \[code\]submit\[/code\] is the submit tagNow I don't know how to connect the \[code\]user\[/code\] with the \[code\]form\[/code\], when I click the \[code\]submit\[/code\], how should I know which user I want to add a tagHi,all I'm new in HTML , so I need your help!
