File Upload Through Html

Me and another member struggled to get html file upload up and working through this tread <a href="http://www.totalchoicehosting.com/forums/index.php?showtopic=8560&hl=" target="_blank">http://www.totalchoicehosting.com/forums/i...wtopic=8560&hl=</a>. I had everything working, and tested it several times. I got a message from one of my users this morning saying that it was not working now. I had not made any changes!
