OR you could just make your own with the Photoshop .ico addon, that lets you save anything in the .ico format.
I'll upload that if anyone wants it.

IconConvert is also good, I use it alot. But it only supports PNG images atm, but I still love it! Anyways, the one you have posted looks great too, I'll add it to my favourites. =P
