Dynamic html,php form

I am creating a website,I would like users to create forms for themselves that is they can add field like name,age ,gender and each form will be different coz each user has his/her own specification.Then after creating the form it is stored in a different file then users of the website can come fill in the specific form which then saves in the database any help
