I am looking for a dedicated managed server - either in the US or the UK.
My budget is around US$400/month. What is the best I can get in this price range (balancing up: internet connection speed/server speed and capacity/service) and what are the companies that can give this?
I need to be able to add extra servers in the coming months.

Definitely sounds doable, you can check the dedicated offers section too, or send an e-mail to a hosting company like 478east and ask for a custom quote on something like that. Can I ask what you plan on using the server for? I would disagree with the other opinions in this thread. Yeah, we have some of our servers there as well (around 50 servers) and they are top notch.
--TinaI think you can get Dual Xeon or Opteron server with enough RAM and 3xHDD (RAID 5). Of course the U.S. service will be less expensive because of the weak dollar. About other things it is better to have a list of requirements and to send it to some web hosting companies.
