Create cookie input prompt

I've created a form.I need to generate a cookie for that form which does the following[*]Onload Prompts for ID, NAME, MANAGER one time during first use. The propmt do not appear again for that user until after the cookie expires [*]Cookie Expiry 365[*]Once the cookie is stores the fields(ID,NAME,MANAGER) on the form are always poulated for that user All assitance is much appreciated
