bb code error

this error appears when i try 2 add new bb code
 
You have a hack installed that already sets the bbcode you are trying to install. Probably a multimedia hack. Disable hooks to see if you can add the bbcode without your hack running. If you can, then you need to manually disable your mods and re-enable them one at a time until you find the one that is the problem.
