Adding check box and Text area in an alert

I have a situation where I have to add a check box and text area in an alert box that pops up after clicking a button.<br />

Is it possible to add these in an alert box?<br />
<br />
If possible can we trap the value of the check box if its checked?<br />
<br />
Any help would be greatly appreciated..<br />
